Tax forms may be printed from the internet.

• Federal URL: http://www.irs.ustreas.gov
• Colorado URL: http://www.revenue.state.co.us/

Tax forms can be printed from any Library computer for 10 cents per page.

Tax Forms are available IN PERSON at:

• Internal Revenue Service, 301 South Howes, 3rd floor (Federal forms)
• Colorado Department of Revenue, 1121 West Prospect (State Forms only)

Tax Forms can be ordered over the PHONE by calling:
